Health Risk Assesment

The Patient Affordable Health Care Act (PPACA) affords Medicare beneficiaries access to Annual Wellness Visits (AWV) with no copayment or deductible. The Annual Wellness Visit (AWV) is a preventive screening encounter aimed to promote increased utilization of preventive services covered under Medicare Part B. The interview encounter with patients involve a comprehensive Health Risk Assessment (HRA). Individuals are provided a complete Health Risk Assessment (HRA), a written screening schedule, outline of risk factors, and a wellness or Personalized Prevention Plan (PPP) AWVs provide an opportunity for primary care providers to improve patient care while simultaneously generating income through Medicare reimbursements.

Chronic Care Management (CCM)

Chronic disease management & Care Co-ordination

Chronic Care Management (CCM) is the non-face-to-face service provided to Medicare beneficiaries who have two or more chronic conditions. CCM covers chronic disease management services, and include communication with the patient and other treating health professionals for care coordination, medication management, and accessibility to providers or other clinical staff.

Transitional Care Management (TCM) Support

Care Co-ordination

TCM allows for services to be provided to a patient whose medical and/or psychosocial problems require moderate or high-complexity medical decision-making during transitions in care from an inpatient hospital setting (including acute hospital, rehabilitation hospital, long-term acute care hospital), partial hospital, observation status in a hospital, or skilled nursing facility/nursing facility, to the patient’s community setting (home, domicile, rest home, or assisted living).
